Disease Reporting

If you are a medical provider or representative, you are responsible for           reporting notifiable diseases to the Montgomery County Public Health          Department.              

To report a case of the flu, please use the Flu Reporting Form.

To report a case of varicella (chickenpox), please use the

Varicella Reporting Form. 
 
To report any other infectious disease, please print the

Infectious Disease Report.

Once completed, disease reporting forms may be faxed to 936-539-9272

or emailed to Syed Ibrahim.
