If the patient is a minor (less than 18 yrs of age) is not accompanied by a parent, please provide proof of legal guardianship, or a letter from the parent/legal guardian giving permission to have another adult consent for the child, along with a copy of the parent/legal guardian's drivers license. Immunizations will not be giving without proof.If you are a student needing a vaccine, please call our office as soon as possible to schedule an appointment so we may better serve your needs in a more timely manner.
