For van service transportation call 936-523-5107:

This service is only for those clients who are active on the healthcare assistance program. The van service only provides transportation to and from medical appointments within Montgomery County. You must call 48 hours in advance to schedule this service. Please be prepared to provide your name, HCAP ID number, and phone number along with the date and time of your appointment.
