This service is only for those clients who are active on the healthcare assistance program. The van service only provides transportation to and from the Lone Star Family Health Center. You must call 24 hours in advance to schedule this service. Please be prepared to provide your name, HCAP ID number, and phone number along with the date and time of your appointment.
