How to Apply for HCAP
To apply for the HCAP, visit 200 River Pointe Dr., Suite 303, in Conroe by the new IHOP. Click Here for a map from Yahoo ®
All applicants will need to fill out a one-page application along with a pre-screen form and pay a $5 processing fee if eligible. Applicants are seen by appointment only, but only after copies of all the required documentation are provided to the HCAP office. For a list of information you will need to provide in order to schedule an appointment (in addition to the application and pre-screen form) use the link below, which will provide instruction on how to apply.
