How to Apply for HCAP

All applicants need to fill out an application and provide all requested supporting documentation. Applicants are seen by appointment only, but only after copies of all the required documentation are provided to the HCAP office. For a list of information you will need to provide in order to schedule an appointment (in addition to the application) use the link below, which will provide instructions on how to apply. A processing fee of $5.00 is charged per applicant found eligible for HCAP.
